Personal Data Sheet and Work Experience Sheet

 If you are applying for any position in the government, you will be required to submit a duly accomplished Personal Data Sheet (PDS) and Work Experience Sheet (WES).

PDS is a form required by the Civil Service Commission to be submitted by a government official or employee before his or her assumption to duty. It is a repository of personal and professional information about an official and employee and more comprehensive than biodata.

CSC also requires WES which shall include the work experiences relevant to the position being applied to.

Even after the appointment or assumption to duty of government workers, they are obliged to submit PDS and WES every year to update their information to CSC.
